29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Ingoing nip point was not guarded on #3 paper machine reel drum and partially built parent reel on August 7, 2013. b) On or about 12/3/2013, ingoing nip points on threading ropes for reel were not guarded. c) On or about 12/3/2013, ingoing nip point formed by idler roll and belts on back side of reel at startup tire was not guarded.
