Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from a mast and/or broken crown block on a well servicing rig. On or about 8/28/13 at a the Gunner #2 oil well located off from Fulmer Valley Road, Andover, New York; employees servicing an oil well were using a shop built pulling rig to pull 2 inch production tubing out of the well. A packer that had been installed would not release and employees continued pulling without knowing the actual capacity of the rig or the amount of force being applied to the mast and crown block. The crown block broke off from the mast striking an employee. ABATEMENT DOCUMENTATION REQUIRED
