29 CFR 1910.23(e)(7)(iii): Every pit and floor opening was not adequately guarded by standard railing or any other effected means. On or about August 27, 2013, and at times prior thereto, the employer did not ensure that the floor opening cover above the mud pit was kept in good repair, exposing employees to the hazards of falls into the mud pit which contained drilling fluids heated to approximately 160 Degrees.
