Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to flash fires and explosions: (a) On or about July 16, 2013 for employees who were not protected from flash fires and explosions by the use of a lockout tagout program while performing maintenance work on a tank battery at the Petro Hunt 150-102-6A-7-1H well site near Alexander, ND. Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to follow the recommended practices identified in the American Petroleum Institutes Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations API RP 54 section 6.9 Lockout/Tagout.
