29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: A.) Thomas Tree Care Professionals, 614 South Walnut Street, Sharpsville, PA 16150, on or about July 11, 2013 and times prior thereto: An employee was performing arborist work from the bucket of a boom truck (Simon-Telelect Hi-Ranger XT55). Employee was not utilizing a fall protection device and fell approximately 50-feet from boom trucks bucket to the ground below.
