29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 26 O'Brien Court, Bayonne, NJ and at the Corporate Offices at P.O. Box 147, Harrison, NJ. The employer did not instruct each employee in the recognition and avoidance of unsafe conditions in that employees were exposed to struck-by and and crushing hazards as OSHA requirements for masonry construction limited access zones or adequately bracing walls over 8 feet were not known or practiced by exposed employees. Violation observed on or about 06/28/2013.
