Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards due to the following instances: a) Rig #4: On or about June 27, 2013, the employer did not ensure that the drilling rig was operational and safe to use by ensuring that all pins were inserted and secured with keepers or other adequate means in areas such as, but not limited to the dog house support frame, A frame legs, racking board. b) Rig #4: On or about June 27, 2013, the employer did not ensure that the drilling rig was operational and safe to use by ensuring that high pressure mud hoses were secured/restrained in place.
