29 CFR 1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training or experience. On or about June 9, 2013, at the job site located at 1001 Rochier Ave in Fayetteville, Arkansas, the employer did not ensure employees required to operate aerial lifts were qualified to operate the machinery. This condition exposed employees to greater than 10 feet falls and electric shock hazards from energized overhead power lines.
