29 CFR 1910.266(d)(1)(ii): The employer did not assure that personal protective equipment, including any personal protective equipment provided by an employee, was inspected before initial use during each work shift: St. John's Harbor, Zarembo Island, AK: On or about 07 June 2013 and times prior thereto, an employee operating a logging boom boat, regulation number WN-7013-GA, was wearing an unapproved USCG personal flotation device. This condition exposed employee to a drowning hazard resulting in a death.
