29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: a) 160 Bankenson Drive, Summerville, Ga; Roof of Building # 3: Two employees tied off their personal fall arrest system at the same time to a DBI SALA Standing Seam Roof Anchor which was only designed for one person to use as an anchor point per manufacturer, exposing employees to a fall hazard of approximately 36 feet.
