OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards when a blind flange was removed from a pipe that was pressurized: a) Boiler Room - On or about 6/10/2013, employees were exposed to an uncontrolled release of pneumatic energy from working on a charged 8 inch water chiller pipe, under approximately 100 psi of pressure, during pressure test operations. An employee was fatally struck by the blind flange when it became dislodged from the coupling as the piping system was not properly depressurized prior to the victim working on the coupling.
