29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section The employer does not ensure that procedures are developed, documented and utilized for the control of potential hazardous energy when employees are engaged in activities covered by this section. a) This violation occurred on or about June 4, 2013 in the press area where Press Brake operators and maintenance employees were exposed to struck-by and caught-in hazards when performing set-up or maintenance tasks on Accurpress Press Brakes 8096, 8097, 7628, 7630 and 9043 without specific procedures to control hazardous energy. b)This violation occurred on or about June 4, 2013 in the press area where Press Brake operators and maintenance employees were exposed to struck-by and caught-in hazards when performing set-up or maintenance tasks on a Steelweld F3-6 Mechanical Press M765 without specific procedures to control hazardous energy. c) This violation occurred on or about June 4, 2013 in the press area where Press Brake operators and maintenance employees were exposed to struck-by and caught-in hazards when performing set-up or maintenance tasks on Accurshear Hydraulic Sheer 4595 without specific procedures to control hazardous energy. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that procedures are developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in activities covered by this section.
