29 CFR 1910.268(c): The employer did not prepare employee training certification records which included the identity of the person trained, the signature of the employer or the person who conducted the training, and the date the training was completed: a) Knollwood Drive and Riverdale Avenue, Tinton Falls, NJ 07724: On or about May 14, 2013, the employer failed to certify that each employee was trained in the recognition, avoidance and mitigation of the hazards associated with telecommunications work. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
