29 CFR 1910.23(a)(3): Every hatchway and chute floor opening shall be guarded by hinged cover(s) or railings: Inside Roof Access. On or about March 6, 2013, the hatchway opening for the fixed inside ladder used to access the roof was not equipped with standard railings , closed cover or equivalent protection to prevent employees from falling through the opening once on the roof, and when pulling tools and supplies up through the hatchway by rope cable while standing on the roof near the opening. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
