29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: (a) 110 Westminster Avenue, Hanover, Pennsylvania - The employer opened an excavation in the front yard of the residence in order to replace the sanitary sewer line from the house to the township sanitary sewer line. The employer did not protect, support, or remove the underground natural gas line to prevent damage to the line. On April 19, 2013, the trench collapsed onto the natural gas line, tearing the gas line loose and creating the hazard of a fire and explosion from a natural gas release. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
