Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees; in that employees were exposed to flash fire hazards. On April 16, 2013, University 42 Lease, Well #1L, the employer did not ensure precautions were taken to maintain well control. Employee on site did not secure the Blowout Prevention (BOP) prior to beginning Hot Water Operations and did not position source of ignition more than 100-feet from the wellhead, exposing employee to flash fire hazard.
