29 CFR 1910.184(c): Whenever any sling was used, the safe operating practices noted in 29 CFR 1910.184(c)(1) through (c)(14) were not observed: a) communication tower at 1004 Point Hill Drive in Franklin, PA. On or about April 4, 2013, the sling used in a basket hitch did not have the load balanced to prevent slippage and the sling was not securely attached to the load. The sling was not rigged properly and allowed the load to fall 140 feet to the ground.
