29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): a. Dry lines - On or about April 4, 2013, lockout devices affixed to the mixer and skip bucket of line #6 did not indicate the identity of the employees performing the clean out servicing tasks. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
