29 CFR 1910.178(l)(1)(i): The employer did not ensure each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about March 26, 2013, and times prior thereto, an employee was allowed to operate the Linde forklifts from the basement area prior to demonstrating competency in operation of the powered industrial trucks. Employee was exposed to crushed-by hazard.
