29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling objects, were not protected by protective helmets: JOB SITE: 1201 S. Fifth Street, Saint Charles, MO 63301, at this work site, a worker was observed inside an open trench, approximately 7 feet 5 inches wide, 15 feet 7 inches long and approximately 6 feet 7 inches deep, actively tapping into the main sewer line and was not protected by protective helmets. The worker was exposed to a potential cave-in hazard.
