29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about March 12, 2013, employees performing masonry stone installation activities from a (6) tier tubular welded frame scaffold that was not equipped with end guardrails. The employees were exposed to fall hazards of up to (21) feet to the ground.
