29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: (a) Fort Beeler Facility, Cameron, WV: The employer did not comply with Recognized and Generally Accepted Good Engineering Practices (RAGAGEP) when it failed to protect employees from fire and explosion hazards inside inadequately protected structures including, but not limited to, the portable lightwood constructed trailer used as the site control room as determined on February 8, 2013. (b) Fort Beeler Facility, Cameron, WV: The employer did not evaluate and document that controls used as safety interlock systems (SIS) such as, but not limited to, PAHH-411A interlock for high pressure in an inlet pre-heater complied with Recognized and Generally Accepted Good Engineering Practices (RAGAGEP) prior to listing this control as a safeguard in the initial Process Hazard Analysis as determined on March 12, 2013.
