Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to crushing hazard: a) On or about 2/01/13 at the production area; an employee, operating an H-25 B payloader, was caught between the bucket support arm and the hood of the payloader. Among other methods, several feasible and acceptable abatement methods to correct this hazard include following the Association of Equipment Manufacturers, Safety Manual for Operating and Maintenance Personnel of Wheel Loader/Tractor, 4/05 revision. 1) Page 7 states: For safe operation of this machine, you must be qualified and to be qualified, you must be trained and know the safety rules. 2) Page 9 states: Whenever you leave the machine, always lower the loader bucket or other attachments. Engage the parking brake, stop the engine, cycle the hydraulic controls and remove the key. 3) Page 21 indicates a Warning: Never let anyone in or near the pivot area of articulated machines.
