29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) On or about November 20, 2012, for the employees standing on top of tanker trucks during loading operations without fall protection systems in place and were exposed to a potential fall hazard of approximately 10 feet, located at 10245 Highway 2 #C, Tioga, North Dakota.
