Text For Citation: 01 Item/Group: 001 Hazard: ELECTRIC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards of electrical shock from defective twist lock plugs and damaged outer metal cables in the following locations: a) On or about 9/10/88 Die Stand b) Welex Extruder (9/14/88) While adjusting, maintaining, servicing or examining plastic extrusion dies heated by 240 volt electric conductors. Among other methods, one feasible and acceptable abatement method to correct this hazard is toestablish a program of physical examination, immediate replacement of damaged "Die Power Leads" and the installation of ground fault circuit interrupters (GFCIs) to protect employees.
