Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) Mold Yard Area; Employees were exposed to being struck by radio remote control diesel locomotives in that operator training was inadequate. There was no formal training by a qualified instructor; there was no documentation of the On-the-Job-Training to indicate content and operator qualification; there were no manual medical evaluation; the locomotive inspection reports were not being done on a daily basis; and there was no monitoring of operator performance. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide proper training on a continual basis with ongoing monitoring and enforcement.
