29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operators performance was not conducted at least once every 3 years: a) ADM Milling Company - On or about November 14, 2012, employees operated powered industrial trucks inside the pack house, loading trailers, and near the maintenance shop. ADM Milling Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 311465157, citation number 01, item 003 and it was affirmed as final order date on 3/26/2009 with respect to a workplace located at ADM Milling Co, 540 South St, Lincoln, NE 68502.
