29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about 12/11/2012 at 654 Univeter Road Canton, GA; the employer failed to ensure that an annual audit of the lockout/tagout program was performed, where employees are required to do maintenance and repair of equipment that included but was not limited to the #1 Hopper and #1 Dumper in the MDM room.
