OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about October 18, 2012 - During work on the Bulb Tee Girder Line Prestress Bed #B4, employees performing a sequence of tasks while moving along a concrete form more than four feet above the ground level with open-sided edges where standard guard rails could not be installed were exposed to a fall hazard.
