29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. (a) Dave Tackett Construction, Inc. on or about October 09, 2012, at the address of 4065 Jaycox Rd. Avon, OH: The employer did not ensure employees were protected from head injuries while working below employees conducting sheeting and roofing work above. Employees were exposed to potential impact injuries caused from falling material and/or equipment.
