29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Construction site storage yard - on or about September 4, 2012, an employee using a CAT 930H loader with a fork attachment to simultaneously lift two concrete road barriers from a truck, with a combined weight of approximately 12.860 lbs., exceeding the loader's maximum operational lifting limit with pallet forks, as specified in the operator's manual an the employee was not sufficiently instructed as to the operational lifting limit of the loader with pallet forks to enable him to recognize and avoid unsafe conditions or otherwise control or eliminate hazards or other exposure to illness or injury.
