Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe struck by hazards when testing new hydraulic cylinders for leakage: The employer failed to ensure all components installed and operated on the old hoist test stand were properly designed, specified and rated for the intended operating parameters when testing new hydraulic cylinders in building AA. On or about August 22, 2012, the hydraulic coupler on the return line catastrophically failed, fatally injuring the employee. The hydraulic coupler on this machine also catastrophically failed in 2008 and 2010.
