29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a) On or about June 28, 2012 and at times prior, Key Energy Services LLC did not ensure that employees were protected from fall hazards. Employees did work, at heights greater than 14 feet, without fall protection. The employees were not provided fall protection systems that would be applicable to the task of lowering the wind walls.
