29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. Hanger Deck, Building 2641, HSC-25, Andersen Air Force Base, Yigo, Guam. On or about July 19, 2012, and at times prior thereto, the employer did not require the wearing of appropriate personal protective equipment including hard hats, eye and face protection, in all operations where there is an exposure to hazardous conditions from falling objects, grinding, and paint spraying operations.
