29 CFR 1910.23(a)(2): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing on all open sides and railings were not provided with a toeboard where required: Cement room: On or about July 12, 2012 and times prior thereto, the skid cement pump work area was 55 inches above the deck with no rail or gate on either side of the platform. This exposed employees to a fall hazard of 55 inches.
