29 CFR 1926.20(b)(4): The employer failed to permit only those employees qualified by training or experience to operate equipment and machinery: Construction Site: On or about July 11, 2012, Employees were not trained to recognize the hazards associated with operating heavy equipment. The employer did not ensure employees were qualified and certified to operate the John Deere 410G-Backhoe Loader(s). One employee was struck-by/caught-between the digger bucket and the stabilizer leg of the 410G-Backhoe Loader.
