29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 2, 2012, an employee was scalped while operating a DAEWOO Puma 200 LC CNC Lathe. The door of the machine was not effective in guarding from hazards associated with the rotating spindle of the machine in that the spindle continued to rotate when the door was opened.
