Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about May 21, 2012 and times prior thereto, at the well site located at or near Dumas #1 and other locations, El Dorado, AR, employees who were assigned duties involving the maintenance, repair or demolition of tanks were not effectively trained in the recognition and avoidance of unsafe conditions. In one instance, three employees performing work at the aforementioned location were fatally injured when a tank containing flammable residue exploded. The failure to adequately and effectively train employees in the hazards associated with storage tank repair, maintenance and demolition exposed employees to fire hazards such as, but not limited to, smoke inhalation and thermal injuries from explosions and/or tank collapse.
