Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) On May 16, 2012 at the job site located at the intersection of 25th Street and Moffet Avenue in Joplin, MO, an employee was exposed to a struck-by hazard in that the employee was using a gasoline powered cut-off machine to make a cut on the exposed east portion of a previously installed 8" cast iron water main pipe without the pipe being supported to help prevent the pipe from shifting during cutting which causes the cutting wheel to be pinched and the cut-off machine to kick-back at the employee. AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE MEANS OF HAZARD ABATEMENT include: The employer can take sufficient steps to help ensure adherence with the requirements in the saw manufacturer's instructions and the employer's own practices, both of which require that the pipe being cut must be adequately supported to prevent the pipe from shifting, pinching the cutting wheel, and kicking back. Abatement certification is required for this item - use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose, and, in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
