Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:06/24/13 Location:Front Parking Lot Equipment:Load Lifter Rough Terrain Forklift, Model #2214-8D, Serial #3396 Condition:An employee installing a hydraulic cylinder on a forklift was exposed to the hazard of being struck or crushed by the unsecured elevated mast and carriage. Among other methods one acceptable means of abating this hazard is to adhere to the manufacturer's warning posted on the equipment not to engage in repair activities on raised equipment unless the attachments are securely blocked. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
