Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: (a) Water jet area- An employee was exposed to a struck-by hazard when a metal high pressure water line coupling failed during the initial start-up of a recently relocated water jet cutting machine. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the ASME Standard B31.3-2010, Chapter VI (Inspection, Examination, and Testing), Paragraph 344.2.1 (Visual Examination of components, joints, and other piping elements including threading.) To correct this hazard, the metal high pressure tubing (piping) and the threading die should have been visually examined and/or inspected for defects, threading dimensions or other component and piping elements prior to installation. In addition to ASME Standard B31.3, follow the instructions in Chapter 9 of the manufacturer's M- 127, Small High-Pressure Components Manual, Revision 1 for coning and threading of high- pressure tubing. This chapter also covers torque specifications and positioning of the high pressure collar contained inside the coupling system. Follow the instructions in Chapter 4 of the manufacturer's manual M-287, Model 7X, Intensifier Pump, Operation and Service Manual, May 2000 which states when pressurizing any new, rebuilt, or serviced high-pressure component, clear all personnel from the immediate area until the system pressure has been applied for a period of three minutes and has been cycled on and off at least three times.
