Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck or crushed by falling concrete. An employee was walking/working below a section of the tunnel where sprayed on concrete was recently applied. The employee was struck by a piece of concrete which dislodged from the ceiling area. Among other methods, feasible and acceptable abatement methods to correct the hazards are to: Comply with ACI 506.2 Specification for Shotcrete section 3 Execution. Section 3.1 Examination Verify surfaces are acceptable and ready to receive work. Verify that surfaces to receive shotcrete have been properly prepared according to the Contract Documents. Section 3.3-Surface Preparation 3.3.2 Concrete, masonry and shotcrete - When bonding is required, remove all deteriorated, loose unsound material or contaminants that may inhibit bonding. Chip areas to be repaired to remove offsets causing abrupt changes in thickness. Taper edges to eliminate square shoulders at the perimeter of a cavity. Surfaces shall be saturated surface dry immediately prior to shooting. Comply with MTA directive The MTA has modified the safe work plan for working with sprayed on concrete. The new rule prohibits employees from standing under freshly poured concrete for 90 minutes. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 PER 29 CFR 1903.19.0
