Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: (a) Location - Urethane Compound Room: On or about 11/16/11, a room with connecting walls and doorways to another portion of the building was used for storage of flammable liquids and dispensing / mixing of Class I flammable liquids. The room was approximately 3500 square feet and did not meet the safety requirements for a cut-off room. Among other means, feasible abatement would be to reduce the amount of flammable liquids stored in the Urethane Compound Room to a combined quantity acceptable for safety purposes; move dispensing and mixing operations out of the room into a 1. Have a qualified ventilation service balance, reconfigure or repair the existing ventilation system so the room is under adequate negative pressure; ensure intakes demonstrate adequate velocity/volume; ensure floor sweeps are not blocked or deficient in their operation in accordance with NFPA 30: Flammable and Combustible Liquids Code accordingly. 2. Apply suitable fire stop material to all wall penetrations and under pedestrian door to revalidate fire rating in accordance with NFPA 30: Flammable and Combustible Code accordingly. 3. Install an approved pedestrian fire door with a minimum rating of 3 hours in accordance with the four hour wall assembly in accordance with NFPA 30: Flammable and Combustible Liquids Code and NFPA 80 Fire Doors and Windows.
