Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards. "On or about December 8, 2011, and at times prior thereto, employees were exposed to struck by hazards while engaged in oil/gas well drilling and/or servicing operations when a section of the de4rrick collapsed and struck and employee. The employer did not ensure the rig mast and elevated hoisting system were adequately inspected prior to use, to identify structural defects." Among other methods, feasible and acceptable means to abate this hazard include: 1) Follow the inspection guidelines identified by the rig manufacture to ensure that an inspection conducted by qualified person includes the mast, crown, draw works, and hoisting system; and 2) Implement inspection procedures in accordance with the American Petroleum Institute RP 4G, Recommended Practice for Use and Procedures for Inspection, Maintenance, and Repair of Drilling and Well Servicing Structures.
