Employees involved in these tasks were exposed to possible serious injuries or death from struck-by, crushing, and suffocation hazards due to the moving conveyor, large volume of material being pushed onto the conveyor by the front- end loader, and potential for being conveyed into the baler. An employee was asphyxiated as a result of being engulfed in approximately two tons of paper products. The decedents body was located on top of mixed paper upon the conveyor. References: 1) American National Standards Institute (ANSI): ANSI Z245.41 - 2008: for Equipment Technology and Operations for Wastes and Recyclable Materials - Facilities for the Processing of Commingled Recyclable Materials - Safety Requirements.- 5.1.1.14: Automatic cycling equipment. Ensuring that processing equipment and systems which are equipped with automatic cycling controls are used only in locations where the loading chamber is not accessible to personnel while the automatic cycle is functioning. - 5.1.1.15: Prohibition from riding on conveyors. Not permitting riding on conveyors in any new facility subsequent to the effective date of this standard. In facilities which were in existence prior to the effective date of this standard, riding shall be permitted only on conveyors which are specifically designed for this purpose, and which have the following characteristics. The conveyor must: a) move on a flat plane; b) not be elevated above floor level; c) be at least 24 inches (610 mm) and width; and d) be equipped with an emergency stop device(s) which is readily accessible by an employee riding on the conveyor. 2) Conveyor manual (make: Mayfran International, Incorporated). - Page 4: SAFETY OPERATION - DO NOT operate any machinery (or any portion of this machinery) unless all personnel are clear of any rotating or moving parts (or parts that may potentially move or rotate). - DO NOT ride or walk on any conveyor. - Page 44: DECAL PLACEMENT (4): 2. DO NOT STAND, SIT, WALK, OR RIDE ON CONVEYOR BELT. One feasible and acceptable means of abatement method is to ensure that employees follow all safety rules on the proper operation of conveyors.
