Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to an unsecured outer elevator doors: (a)On or about 10/06/11 - Feed Mill, the elevator doors interlocks on several landings did not work allowing exposure to a fall from the 6th floor to the basement level or to be struck/crushed by the traveling car. Reference ASME A17.1-2007 Safety Code for Elevators and Escalators section 5.7.9 Among other methods, one feasible and acceptable abatement method to correct this hazard is to repair the existing interlock on each floor.
