Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to carbon monoxide exposure and fire hazards: a)On or about 7/7/11 to 10/18/11- Boiler room, the relief stack connection between the furnace and the boiler had visible holes and a deteriorated weld exposing employees to hot ash and gases. Among other methods, one feasible and acceptable method to correct this hazard is to properly, inspect, maintain and repair equipment in accordance with NFPA 211-2009, "Standard for Chimneys, Fireplaces, Vents and Solid Fuel-Burning Appliances.
