Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of fire and bodily injury: (a) Location - Bradford clean utility room: On or about October 25, 2011, a cylinder of compressed oxygen gas in storage inside a utility room was not secured to prevent being knocked over. Among other methods, feasible means of abatement would be to secure cylinders in storage as described in Compressed Gas Association Pamphlet P1-2006, Section 5.7.4.
