Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) First floor elevator, on or about December 29, 2011, the elevator door on the first floor did not have an operable interlocking device that would prevent access to the elevator shaft when the elevator car was in use and/or at the third floor level. Employees were thereby exposed to fall hazards associated with the shaft opening. A feasible method of abatement is to install electrically and mechanically interlocked gates at all levels with an additional gate to guard the opening below the car/lift table when it ascends to upper floors, in accordance with ANSI A17.1, Sect 204.4b and ASME A17.1 Rule 111.5. a)First floor, back room, on or about 12-29-11: One employee was operating an elevator that was not equipped with an operable interlocking device, exposing the employee to a fall hazard of 10 feet 3 inches. Abatement certification is required for this item.
