Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards on or about June 2, 2011. a. At the warehouse: Employees were lifted to or lowered from the file aisle using wooden platform on the forks of powered industrial trucks. Employees were exposed to a 8'-4" fall. Acceptable methods to correct this hazard are: 1. Install a permanent ladder or use of a portable one, or 2. Use of elevating personnel platform according to ANSI/ITSDF B 56.1-2005, Section 7.37 Platforms: Elevating b. At the warehouse: Employees were lifted to or lowered from the file aisle using manmade wooden platform on the forks of powered industrial trucks without the use of fall protection. Employees exposed to 8'-4" fall. Acceptable methods to correct this hazard are: 1. Use of personal fall arrest systems when an approved elevating personnel platform is being used.
